Changes to Dismissal 

For the 2025-26 school year, Evans Park Elementary will continue to utilize a Google Form system for dismissal changes.  If your child will have a dismissal change, kindly complete the DAILY DISMISSAL CHANGE FORM before 11:30 on the day of the dismissal change.

Any change to dismissal after 11:30 AM should be called into the main office at 845-620-3950

In accordance with New York State regulations, student physical examinations are required for all students in grades K, 1, 3, 5, 7, 9 and 11 and also for all students new to the Pearl River School District. We continue to recommend annual physical and dental examinations for all students. Physical examinations must be done within 12 months of school entry in order to comply with State requirements for the upcoming school year. Please note that the physical form provided on the link below is the ONLY FORM that will be accepted. A physical dated after September 2, 2024 will fulfill NYS requirements.

Students who are unable to obtain a physical examination from their primary health provider should contact their school nurse. Students who are under-immunized and/or do not have appropriate immunization documentation on file in the health services office, will not be able to continue to attend school. Please see link below for required immunizations according to grade level. Required physical examinations and immunizations must be submitted by September 1, 2025.
